I got locked out of my device which is not activated anymore, is not linked to my google account and i cant remember the phone number i had when it was active so i cant log into my verizon account. Can you help me?

Tried messaging Verizon Wireless through their Facebook page to see if Customer Care would respond
Was equipped with the following information
Email, First name, Phone Number on Account, Email on Account, Last name, Home address, Billing Address on Account
